  1. Captains, please ensure all your team members have registered on the 10,000 steps website (if not done so previously) under “Join the step log”.
  2. To be eligible for entry into the early bird prize draw, this form with all member’s details, must be submitted by Friday, 18th September 2009.
  3. Registrations close on Wednesday, 23rd September 2009.  Late entries may not be processed in time for the start of the challenge.  
  4. After submission, Team Captains will receive a confirmation email and further instructions.
